Services for Spouses

Authorization Form

Spouses of students may also receive services from the Student Health Center. Here’s how your spouse can obtain those services:

• You and your spouse must be legally married and you must be a currently enrolled student.

• You must sign the attached authorization form, allowing the following charges to be billed to your student account:

- A pre-paid health fee of $125 for your spouse, and

- Charges for any services that your spouse receives that are not covered by the pre-paid health fee. The services that are covered are described below

• If you are a part-time student, you must also purchase the pre-paid student health fee to receive Student Health Center services for yourself.

After you take the steps described above, a $125 pre-paid health fee will be charged to your student account and your spouse will be able to receive services from the Student Health Center beginning two weeks before the first day of classes for the semester and ending at the close of business on the day prior to classes for the following semester. For your spouse to receive services again in the next semester, you will need to repeat this process, including signing a new authorization.

The services that your spouse can receive in exchange for the pre-paid health fee will be the same as those listed in the guide for the student pre-paid health fee, with the exception of mental health services. For those services, your spouse will receive only one visit for an assessment and a referral to community mental health services for all future mental health care. If your spouse receives services that are not covered by the pre-paid health fee, the charges for those services will be billed to your student account unless they are paid at the time of service by cash, check, or credit card.

If your student account becomes inactive for any reason or if you cease to be legally married, your spouse may still receive the services covered by the pre-paid health fee for the rest of the time period covered by that fee, but if your spouse receives services that are not covered by the pre-paid health fee, he or she will not be able to bill the charges for those services to your student account. Instead those charges will have to be paid at the time of service by cash, check, or credit card.
